  • This plan is only honored at Coffee Creek Family Dentistry. It cannot be used at any other dental or specialist office.
  • This plan cannot be used for hospitalization or hospital charges of any kind.

  • Any unused services within the patient membership are nontransferable to other plan members regardless of the service.
  • Any missed appointments or cancellations not made within 48 hours of the appointment may be charged a cancellation fee.
